How to apply attendance report and academic transcript for visa renewal?

1.Visit our portal at link : https://utpmy.sharepoint.com/sites/UTPCGSFORMS
2.Scroll down and select form 004. (REQUEST OF RECOMMENDATION / GA CERTIFICATION LETTERS).
3.Fill up all the details.
For visa renewal, there will be 2 upload part; i.e. Copy of Student Pass/Visa and Attendance Record. You may attach form that has been approved by your supervisor.You may find link for you to download the template.
4.Upon submission of your application, you will be notified by the system on the status of your application.
