How to Apply for A Contractor/Vendor Card.
 

  1. Contractors/Vendors is required to attend UTP HSE Induction Course, which is available every Tuesday and Thursday in Seminar Room 3 and 4, Undercroft.

   Kindly liaise with En Mohd Nazren bin Mohd Zahari (HSSE/UTP) at nazren.zahari@utp.edu.my or 05 3688267.

  1. Contractors/Vendors is required to submit their application via the Unified Customer Service (UCS) Portal for approval. Kindly refer to the guideline provided below.

  • Responsible Department in UTP
  • Person to contact (PIC in UTP)
  • Period of contract (starting to end date)
  • Workplace
  1. Attach files:
  • UTP HSE Induction certificate
  • IC / Passport
  • Copy of contract rewarded. 
  1. Please upload your passport-sized photograph
    • BLUE background
    • Not wearing a hat and the hair does not protect the face and does not cover the forehead
    • Use collared shirts for men
    • Photo captured within 6 months to reflect your current appearance
    • The face must be looking straight at the camera

 

  1. Terms and conditions
  • The payment (RM50.00) each can be made online as below;
  • Universiti Teknologi Petronas, Bank Account No: 8004857910 – CIMB.
  • For reference: Please mention the Company Name and the title being Payment of Contractor/Vendor.
  • Please attach the transaction/bank-in-slip upon settlement of Contractor/Vendor in the UCS Portal. 
  • Contractors/Vendors pass always needs to be displayed in the campus area.
  • Contractors/Vendors is required to abide by all rules prescribed by the University and all laws enforced by the Government of Malaysia. The consequences of breaching any rules are as follows:
  1. Summons will be issued.
  2. Banning from entering the campus.
  1. For renewing Security Pass, Contractor/Vendor shall follow the item no. 1 until 4.
