HOW TO APPLY FOR THE STUDENT MATRIC CARD (STUDENT CARD)
Please submit your request through the Unified Customer Service (UCS) Portal. You may refer to the information provided as below:
- Title: New Matric Card
- Type: Request
- Subject: SSD – Student Matric Card Application
- Your Enquiry: (Please provide the following information)
- Full Name (as per IC/Passport)
- Preferred Name to be displayed on the card (cannot be more than 10 characters)
- Student Level (Foundation, Undergraduate, Master or PHD)
- Matric Number
- Village/Home Address
- IC/Passport Number
- Contact Number
- Intake Year
- Intake Month
- Course/Department
- Gender
- Citizenship
5. Attach file:
UTP Offer Letter
- Please upload your passport-sized photograph
- Blue background
- Not wearing a hat and the hair does not cover the face and the forehead
- Use collared shirts for men
- Photo captured within 6 months to reflect your current appearance
- The face must be looking directly at the camera
7. The initial Matric card is free; however, subsequent cards will be charged as follow:
- Replacement of damaged card is RM50.00
- RM50 penalty will be imposed on the lost card.
- The payment can be made online as below:
- Universiti Teknologi Petronas, Bank Account No: 8004852703 – CIMB.
- For reference: Please mention the Matric ID and the title 'Payment of Matric Card'.
- Please attach the transaction/bank-in-slip upon settlement of the Matric Card in the UCS Portal.
- The photo session and the issuance of the matric card are to be done simultaneously. It is conducted at the Security Counter (One Stop Centre), Chancellor Complex as the per schedule below.
