HOW TO APPLY FOR THE SECURITY PASS (STAFF CARD)
Please submit your request through the Unified Customer Service (UCS) Portal. You may refer to the information provided as below:
- Title: Staff Card
- Type: Request
- Subject: SSD – Security Pass Application
- Your Enquiry: (Please provide the following information)
- Full Name (as per IC/Passport)
- Preferred Name to be displayed on the card (cannot be more than 10 characters)
- Appointment/Job position
- Staff ID Number
- Full Address of current residence
- IC/Passport Number
- Contact Number
- Year of appointment
- Start date and month of appointment.
- Last date and month of appointment
- Department
- Gender
- Citizenship
- Attach file:
Offer Letter/Appointment letter
- Please upload your passport-sized photograph
- Blue background
- Not wearing a hat and the hair does not cover the face and the forehead
- Use collared shirts for men
- Photo captured within 6 months to reflect your current appearance
- The face must be looking directly at the camera
7. The initial Security pass is free; however, the subsequent cards will be charged as follow:
- Replacement of damaged card is RM50.00
- RM50 penalty will be imposed on the lost card.
- The payment can be made online as below:
- Universiti Teknologi Petronas, Bank Account No: 8004857910 – CIMB.
- For reference: Please mention the Staff ID and the title 'Payment of Security Pass'.
- Please attach the transaction/bank-in-slip upon settlement of Security Pass in the UCS Portal.
- Please proceed to the Security Counter (One Stop Centre), Chancellor Complex for the Security pass arrangement. Counter operation hours are as per below.
